This Court has jurisdiction of the parties and subject matter pursuant the laws of

Petitioner and Respondent are both actual and bona fide residents of
________________ County, State of Utah, and were for more than three (3) months immediately prior to the commencement of this action. 3.
Petitioner and Respondent are husband and wife, having been married at

The marriage between Petitioner and Respondent is irretrievably broken and there
are irreconcilable differences between Petitioner and Respondent, making it impossible for the marriage to continue.

There were no children born to or adopted by the Parties. Wife is not now

Petitioner and Respondent separated on ________ day of ____________, 20___,
and from that date up to the present, Petitioner and Respondent have lived separate and apart without any cohabitation. The parties would show that they are entitled to a decree of dissolution of marriage on the grounds of irreconcilable differences pursuant to Utah Laws, Section 30-3-1 (3)(h). The marriage between Petitioner and Respondent is irretrievably broken and there are irreconcilable differences between Petitioner and Respondent, making it impossible for the marriage to continue. 7.
There is no reasonable expectation that the Parties will reconcile.

All matters involving property of the marriage, both real and personal, have been
resolved and the terms agreed to in the Separation and Property Settlement Agreement entered into by the parties. All debts have likewise been resolved by the parties in the Separation and Property Settlement Agreement. 10.
