TLC Equestrian Center, LLC 2361 Rutland Road, Davidsonville, MD 21035 2012 LIABILITY RELEASE & LESSON/SHOW AGREEMENT Riders Name: _________________________________ Address : _________________________________ _________________________________ (_______)________________________ _________________________________ Riders Cell: Riders Email: Date of Birth: ____________ Age: _____________ If rider is under the age of 18: Parents Name: _____________________________ Home Phone: (_______) ____________________ Parent’s Cell: (_______)____________________ Parent’s Email: _____________________________ As a rider and/or boarder and/or parent understanding and agree to the following: WARNING: Please be advised that HORSES ARE UNPREDICTABLE by nature; that when frightened or angry or under stress, a horse’s natural instinct is to jump forward or sideways, to run away from danger, to kick, to buck, to rear up in front, or to bite; that horses are extremely powerful; and that if rider falls to the ground, the fall distance will generally be from 3.5 to 5.5 feet and at speeds up to 35 mph. Riders ride at their own risk, on a voluntary basis, and assumes these risks and dangers. Riders are required to be covered under accident/medical insurance at all times while riding at TLC Equestrian Center, LLC. BARN RULES: 1. Riders are required to wear a certified helmet and paddock boots. Please do not run, shout or toss things around the horses or riding arenas. We ask that everyone clean up after themselves and their horses prior to leaving the facility. Jumping is allowed in lessons or with an instructor present only. Riders agree not to ride when there is no other person on the premises. 2. Horses should not be left unattended while tied, cross-tied or while grazing. Ground tying is not allowed. Horses should be led with a secured fastened halter and lead rope or a bridle at all times. 3. NO SMOKING, drinking of alcohol or drug use is permitted on the property. Stealing, flagrant damage/ destruction of property, abuse of animals, and physical/verbal abuse of personnel or clients will not be tolerated. 4. Clients are responsible for the behavior and safety of their guests. Guests must wear closed shoes while visiting the barn. 5. The maximum speed limit on the property is 10 m.p.h. Please park in designated areas. 6. All visiting dogs must be current on their rabies vaccine and be kept on a leash at all times. Owners of said animals accept full responsible for the animal’s behavior and actions. Owners must clean up after their dogs. 7. All riders must be covered by accident/medical insurance while being enrolled in a lesson semester. 8. With the exception of summer camp, minors must have a parent present to supervise them at all times outside of the minor’s lesson time. TLC has my permission to use photos taken of rider (students & boarders) in advertising & on website. 9. TLC Equestrian Center, LLC, Terry Cash, Sherry Cash, and/or employees are not responsible for any personal items or equipment left on the property. LESSON POLICY: 1. Lesson fees are due on or before the last day of the month for the following month’s lesson semester. A $25 late fee will be charged when payment is paid on or after the 1st. 2. A make up will be awarded when the instructor cancels a lesson or when the instructor is notified by noon of a student’s absence. Students and/or parents agree to keep track of make up lessons. Make up lessons do not expire and can be made up as long as the student in enrolled in a paying semester. Lesson credits can be applied toward lessons only. No refunds will be given for lessons not taken. 3. During weather that is not suited for riding, lessons on horsemanship, horse and tack care, etc. may be offered in lieu of riding. Riders who choose not attend the indoor lessons agree to forfeit a make up lesson credit with the exception of those who have mastered the topic and have been excused by the instructor. I, the rider/boarder/student/parent and undersigned, acknowledge that this agreement is entered into in the State of Maryland and will be interpreted and enforced under its laws. I, the undersigned, being of legal age and of sound mind and not being under the influence of drugs or intoxicants, have read and understand the foregoing agreement and release. I also acknowledge receipt of a copy of this agreement and am in full agreement with TLC Equestrian Center, LLC’s rules and liability release, lesson and horse show policy signed and/or posted on the premises.
