How To Redeem Your Rebate 1.
You must enclose all of the following items in order for your Rebate to be properly processed: a. This completed Rebate Form. Keep a copy for your record. b. A copy of your Sales Receipt (or invoice) with the qualifying MSI VGA card purchase circled. Keep a copy for your record. Your sales receipt/invoice must show that you purchased the qualifying MSI VGA card between July 01, 2007 and July 31, 2007. Keep a copy for your records. Only one rebate will be honored per receipt/invoice. c. The Original UPC Label from your qualifying MSI motherboard packaging. Keep a copy for your record.

Handwritten invoices or sales history receipts will not be accepted. You must include the original UPC label or the Rebate will be ineligible. No reproduction of UPC label accepted.

Dealers, distributors and other resellers are not eligible for this offer. Offer limited to one rebate per MSI part number/name/receipt/invoice/UPC label. MSI is not responsible for lost, late, illegible, misdirected or postage due mail. Any requests postmarked or received after applicable dates will be returned as ineligible. Offer good in US ONLY.
