South Carolina Department of Motor Vehicles POWER OF ATTORNEY AUTHORIZATION FORM MC-25 (Rev. 6/10) Licensees are required to file IRP/IFTA applications/returns and pay taxes as owed. They are also required to accept and respond to various types of official communications with the South Carolina Department of Motor Vehicles. If a licensee prefers an appointed individual or Service Provider to fulfill these responsibilities, this form is to be completed. This is a privilege extended to the licensee which requires special handling by the Department, therefore, such action will not be considered unless this form is properly completed and placed on file with the Department. The completion of this form does not relieve the licensee of the legal obligations associated with a particular license. The licensee is ultimately responsible for the payment of taxes as well as all acts and omissions of the stated individual or Service Provider. The submitter must ensure that such authentication is proper under the law of that state to authenticate documents sent across state lines. Forms without such authentication will be returned without action. South Carolina Department of Motor Vehicles POWER OF ATTORNEY AUTHORIZATION FORM MC-25 (Rev. 6/10) INFORMATION SHEET A Power of Attorney (POA) is required when someone other that the IRP, IFTA, COC or UCR account holder is given permission to conduct business on the behalf of the account holder. The Power of Attorney is a vital tool in protecting the account holder from someone conducting business in the account unknowingly. Motor Carrier Services will only accept the new Power of Attorney form, (SCDMV form number MC-25). A copy of the form can be obtained at www.scdmvonline.com. Once you are in the SCDMV website, on the left side of the screen scroll to Forms and Manuals. Click on Forms and Manuals and go to the middle of the screen and scroll down to the section Motor Carrier Services. The MC-25 Power of Attorney form is located under Motor Carrier Services. You must indicate the name of the individual on the line to the right of Firms Name that you are giving your permission to for conducting business on your behalf. For example, if a spouse is sent to conduct business and the spouse is not on your account, a Power of Attorney is required. If you obtain a permitting agency to conduct business for you, a Power of Attorney between you and the permitting agency is required. If an individual from your trucking company is to conduct business in the account, indicate the individual’s name on the line to the left of Firms Name. It is the responsibility of the customer to keep the Power of Attorney current. Should there be changes to your selection you must file a new POA with Motor Carrier Services.
